Monitor survey

To be completed annually by suitably qualified persons.

The aim of the following procedures is to provide assurance that the monitor towers are operational and ready for use. Test results shall be recorded electronically via the form below and stored online so that they may be accessed at any time. Maintenance work will not be considered complete until the form below has been successfully submitted and received by the Maintenance Supervisor.

Section 1 - Pre Survey Checks
1.1 Prior to lowering the monitor towers, check each monitor's horizontal and vertical movement limits are appropriate for coverage of the ship's side, nearside manifold and the MLA bund area. Make any limit adjustments necessary following the procedure set out in the Skum FJM100 manual once towers are lowered.
Section 2 - Lower monitor towers one at a time
2.1 Open drain valve and drain tower
2.2 Attach 10 tonne shackles to the base frame and tower anchor points and a 20-30 tonne chain block to the shackles. These should be certified in accordance with local HSE requirements
2.3 Take all appropriate safety precautions to prevent persons entering the area where the tower will be lowered
2.4 Take up the slack before removing the securing bolts
2.5 Apply force to start downward rotation, if necessary use a small block and tackle or lever block and the attachments on the other side of tower to overcome friction or wind resistance
2.6 Operate the chain block to lower the tower until the prop can be lowered and support the full weight of the tower
Section 3 - Monitor checks
3.1 Inspect the monitors for damage and corrosion
3.2 Clean and repair as necessary, and clear any blockages to the nozzle
3.3 Liberally apply grease to all monitor grease nipples
3.5 Clean and repair electrical junction box as necessary
3.4 Open the electrical junction box and examine for water damage
3.6 Remove the Victaulic coupling halves forming each tower's main rotational joint and liberally apply Victaulic brand joint lubricant inside and outside of the gasket before reinstating the joint
Section 4 - Raise the towers and secure in upright position
4.1 Operate the chain block to raise the tower, folding and securing the prop as soon as it is free
4.2 Once raised, secure in the upright position by bolting the lugs together
4.3 Check drain valve is closed and remove chain block and shackles
Section 5 - Service Records
Defect Type: 1 - Critical Defect, 2 - Non-Critical Defect, 3 - Non-Conformance, 4 - Recommendation
